As a courtesy, we submit dental insurance claims for our patients
Please provide your dental insurance card for the most accurate:

- estimates
- co-payments
- Co-payments DUE AT THE TIME OF SERVICE

Full payment is expected at the time of service

- - 5% Courtesy Discount for full payment (Cash or Check) at the time of service for treatment over $800
Appointment Cancellation
- Kindly provide 48-hour notice.
- If less than 48 hours’ notice, a fee may apply.
